Beawell Beauty & Wellbeing Membership Consent
1. Membership Overview
By signing below, I agree to enroll in the Beawell Membership Program. I understand that this membership is designed to provide consistent wellness benefits and requires a minimum commitment of six (6) months.
2. Payment Authorization
Recurring Charge: I authorize Beawell to charge my credit/debit card on file every 30 days for the monthly membership fee of $__________.
Billing Cycle: My first payment is due today, and subsequent payments will be processed automatically on the same date each month (or the nearest business day).
Card Updates: I agree to provide updated card information if my current card expires or is cancelled to avoid service interruption.
3. Six-Month Commitment & Cancellation
Minimum Term: This agreement remains in effect for an initial term of six (6) months.
Month-to-Month: After the initial 6-month term, my membership will automatically renew on a month-to-month basis at the then-current rate unless I provide written notice of cancellation.
How to Cancel: To cancel after the initial term, I must provide written notice at least [30] days prior to my next billing date via email to info@beawellco.com or through the member portal.
4. Member Benefits & Policies
Service Credits: I understand how my monthly credits/discounts work according to company policy.
No-Show/Late Policy: Standard cancellation and no-show policies apply to all appointments booked under this membership. Membership does not waive fees for missed appointments.
Price Adjustments: Beawell reserves the right to adjust membership pricing with [30 or 60] days' advanced written notice.
